PVTA Photo IDs
| You are eligible to ride the bus for half-fare if... |

| a.) You have a Medicare card |
| b.) And/or you are over 60 years of age |
| To obtain an Elderly and Mobility Impaired Transportation Access Pass (Photo ID), you will need to present one of the following: |

| a.) A current copy of your SSI Award Letter (Income Verification for the current year), or a valid Medicare Card (not MassHealth) |
| b.) A Valid Handicapped Placard |
| c.) Be 60 years old with proof of age |
| d.) Be a Veteran with 70% or more of a disability. |
If you cannot meet any one of those requirements, you may fill out a Transportation Access Pass Application. It must be signed by your attending physician.
